PHOTOGRAPHY/VIDEO RELEASE AGREEMENT 

COMPANY: Power Nation Productions, LLC 

PROJECT: Social Media and Advertising Campaign / Toned in 30 Test Group Challenge

MATERIAL: Before and After Photos, Written and/or Videos Testimonials

I, on my own behalf and on behalf of my heirs, successors and assigns, hereby give Company, and its respective affiliated companies, licensees, agents, successors and assigns (the “Licensed Parties”), the absolute and irrevocable right and permission, throughout the world, in perpetuity, with respect to the photographs(s), videography, cinematography, or recording(s) in which I appear, (collectively, the “Material”), taken or made in connection with the creation of content for the Project: (i) to edit, alter, use, reuse, publish and republish the same in whole or in part, individually or in conjunction with other materials, film, television, video tapes, audio tapes, and digital media, in any and all manner, media and channels of distribution known and unknown, including social media, in connection with the Project, including the promotion thereof, and in connection with any other products or services marketed together therewith or as extensions, additions, improvements or successors thereto; and (ii) to use my name, image, photograph, likeness, voice, and signature in connection with the Material and the Project. 

I agree that the Material will be the sole and exclusive property of the Licensed Parties, and I hereby assign all right, title and interest to said Material to the Licensed Parties and agree to execute any additional documents required to evidence such assignment. Without limiting the foregoing, it is specifically understood and agreed that I will retain no ownership rights whatsoever in or to the Material. 

I understand that no monetary or other consideration will be paid or rendered for my services and the rights of use granted hereunder. I waive any right to royalties or other additional compensation arising from or related to my services or the use of the Material. 

No content, advertisement or other material need be submitted to me for any approval and I hereby waive any right to inspect or approve the finished Material, and any video, film, digital media, or printed or electronic matter that may be used in conjunction with it now or in the future, whether that use is known to me or unknown. 

I release and discharge the Licensed Parties from all claims and demands arising out of or in connection with the use of the Material. Nothing herein above will constitute any obligation on the part of the Licensed Parties to make any use of any of the rights set forth above. 

By checking the box at the time of sign-up for participation in The Toned in 30 Test Group Challenge, I hereby agree to willingly grant all rights listed herein above.
